Fern & Fathom Mental Health Solutions, PLLC offers virtual mental health services across multiple states. We provide psychiatric evaluation and diagnosis, medication management (excluding controlled substances), supportive psychotherapy, psychoeducation, and wellness coaching. Our approach includes collaboration with primary care providers and other healthcare professionals in accordance with HIPAA. We encourage integrative approaches that incorporate time outdoors, mindfulness in natural settings, and holistic well-being strategies.

Our Experience
Dr. Raven Aleman DNP, PMHNP-BC, AAT-I, is a Navy Veteran, Psychiatric Mental Health Nurse Practitioner, and Assisted Animal Therapy Interventionist based in Texas, and has been practicing since 2023.
Her career in healthcare began as a Radiologic Technologist certified in Computed Tomography before transitioning into nursing. As a Registered Nurse, she served in various departments, including the emergency room during the COVID-19 pandemic—an experience that sparked her passion for mental health.

Dr. Raven holds a B.S. in Psychology from Texas A&M University, a B.S. in Nursing from UT Arlington, and a Doctorate in Nursing Practice from New Mexico State University with a focus on Psychiatric Mental Health.
She brings a collaborative, patient-centered approach to care, empowering individuals to actively participate in their wellness journey. With a deep respect for integrative and holistic methods. Dr. Raven believes medication isn’t always the only solution. She emphasizes clear communication and enjoys helping patients understand their symptoms, diagnoses, and treatment options.

Outside of her clinical work, she enjoys traveling, fitness, spending time with family, and sharing space with her ever-growing crew of pets—who will often join us during telehealth sessions!
What is a PMH-APRN/PMHNP-BC
A Psychiatric-Mental Health Advanced Practice Registered Nurse (PMH-APRN) is a highly trained nursing professional with advanced education at the graduate level. These providers deliver comprehensive mental health care across all stages of life, offering assessment, diagnosis, and treatment for individuals and families experiencing psychiatric conditions or substance use disorders. They are also instrumental in prevention efforts for those at risk of developing such conditions. PMH-APRNs combine clinical expertise with a holistic approach, prescribing medications and offering therapy in various formats, including individual, group, and family sessions. Their care model emphasizes collaboration with patients, supporting them in achieving personalized goals for recovery and long-term well-being.
